Weight Management Disease Management Program Michigan ranks 6th in the U.S. in highest rate of adult obesity at 25.3% as reported in 2005. In June of 2005, the Obesity Subcommittee of the Cardiovascular Health Advisory Committee released a five year plan to address this issue titled "Preventing Obesity and Reducing Chronic Disease". This report identified that the prevalence of obesity has doubled over the past two decades among Michigan adults. This report also identified some trends that are of concern. Lower incomes were associated with higher prevalence of obesity, and nationally, women of lower socioeconomic status were about 50% more likely to be obese than those of higher socioeconomic status. HPM's Weight Management Disease Management program is designed to work in collaboration with physicians to assist members in achieving and maintaining a healthy weight. Our program emphasizes member self-care and lifestyle changes. Additionally, educating members about their condition and encouraging them to become more involved in the management of their care is an important part of the program. By achieving and maintaining a healthy weight, a member may prevent or slow the progress of many complications such as diabetes, HTN, stroke, dyslipidemia, cardiovascular disease, and certain kinds of cancers.
