Cardiovascular Disease Management Program

Cardiovascular Disease (CVD) is the number one cause of death in Michigan and has been nationally every year since the early 1900's. Michigan has the 6th worst age-adjusted CHD death rate of the fifty U.S. states.

HPM's CVD Disease Management program is designed to work in collaboration with physicians to assist members in focusing on key cardiac risk factors: high blood pressure, high cholesterol, smoking and weight. By encouraging members to become more involved in the management of their care and learning lifestyle choices that lead to better outcomes, we support the physician plan of care. By "treating to goal" providers can enhance outcomes and prevent further complications associated with CVD.

Our program assists providers by:

  • Using all available databases to identify eligible members and assess risk factors
  • Stratifying members to identify outreach opportunities
  • Providing physicians with member updates, Health Risk Appraisal (HRA) reports, and HEDIS updates (use of Medications after a Cardiac Event, Controlling BP, Cholesterol Management)
  • Streamlining the referral and authorization process for needed referrals at the discretion of the primary care physician
