Diabetes Disease Management Program

Approximately 593,000 persons in Michigan have been diagnosed with diabetes that represent 8% of our total population. In 2004, direct medical costs attributable to diabetes in Michigan were $4.5 billion. Although diabetes can not be cured, it can be managed.

HPM's Diabetes Disease Management program is designed to work in collaboration with physicians to assist members with achieving and maintaining blood glucose control with an emphasis on self-care and complication prevention. Additionally, educating members about their condition and encouraging them to become more involved in the management of their care is an important part of the program. By obtaining control of blood glucose levels, a member may prevent or slow the progress of many complications of diabetes such as eye disease, kidney disease, amputations, nerve disease, cardiovascular disease, stroke, flu and pneumonia- related deaths.

Our program assists providers by:

  • Using all available databases to identify eligible members and assess risk factors
  • Stratifying members to identify outreach opportunities
  • Providing physicians with member updates, Health Risk Appraisal (HRA) reports, and HEDIS updates
  • Streamlining the referral and authorization process for needed referrals at the discretion of the primary care physician
