Asthma Disease Management Program

More than 15 million people in the U.S., and more than a half million people in Michigan have asthma.

HPM's Asthma Disease Management program is designed to work in collaboration with physicians to assist members with achieving and controlling their asthma with an emphasis on self-care and complication prevention. Additionally, educating members about their condition and encouraging them to become more involved in the management of their care is an important part of the program. Medication adherence and compliance is a key component in achieving and maintaining asthma control.

Our program assists providers by:

  • Using all available databases to identify eligible members and assess risk factors
  • Stratifying members to identify outreach opportunities
  • Providing physicians with member updates, Health Risk Appraisal (HRA) reports, and HEDIS updates (use of Controller vs. Reliever Medications)
  • Streamlining the referral and authorization process for needed referrals at the discretion of the primary care physician
  • Offering full coverage for asthma education classes for members
