Health Plan of Michigan's Disease Management Program goal is to help members manage their health.All members are eligible for our Disease Management Programs. Once a member is identified on our disease management registry, he/she is enrolled in our program.

By telephone and personalized educational programs, we teach members to:

  • Learn more about their condition and the risks
  • Change the lifestyle choices that could make their condition worse
  • Learn how to become better at taking care of themselves
  • Communicate better with their doctor
